CONTRACTOR ENROLLMENT APPLICATION
REQUIREMENTS
- Active State Contractor's License in good standing with applicable State Licensing Agency
- Active general liability insurance policy with limits no less than $1,000,000
- Satisfactory rating of "B" or better with the BBB (if not listed with BBB other reputation sites will be reviewed)
- Company appearing on State Contractor license must be licensed for at least 1 year or qualifying owner/executive on State Contractor license must be licensed for at least 3 years.
- Active Secretary of State license (corporation or LLC requirement only)
- Active Seller's Permit (sole proprietor requirement only, if applicable)
- Company name appearing on all requirements above must match the company name provided in this application
